In addition to using this form to order reprints, it is to be used to figure out any additional publication fees that your article has incurred. Publication fees include, color separation charges and/or page charges.

Reprints and invoice are shipped separately approximately 4 weeks after publication.
Book reprints, 2-color journal articles, and online articles are printed in black & white.
Orders for articles containing 4-color MUST be received before journal goes to press.
Payment must be received before reprints can be shipped. Purchase orders are
accepted for orders billed to a U.S. address. A pro-forma invoice will be issued for
orders received without payment.
