These guidelines should apply to both PC and Macintosh users.

	Create your list in Microsoft Word. If you do not have MS Word, contact your managing editor for appropriate alternatives.
	Use 12 pt. Times or Times Roman typeface.
	DO NOT format your list with ANY bold type, underlining or italics. Submit the information in plain text ONLY.
	Left-justify the list.
	DO NOT create columns or tables.
	DO NOT bullet or number the list.
	Separate each contributor and his/her information with ONE hard return.
	Separate each line within each contributor listing using ONE soft return (shift + enter)
	For each contributor we will need full name and credentials, home address (unless contributor prefers to receive all correspondence elsewhere), telephone number and e-mail address (if available).
