This CME activity is intended for physicians with an interest in improving their diagnosis, management and treatment of patients.
The United States has approximately 600,000 physicians engaged in patient care to serve its population of approximately 300 million people. The healthcare terminology SNOMED CT includes almost 80,000 concepts with the semantic category of “disorder” in their fully specified names. Collectively these 600,000 physicians therefore need to be able to manage 80,000 disorders.
Given the magnitude of this task, it is not surprising that practicing physicians have serious unmet information needs. Questions arise in as many as one in three patient visits and at least half of these questions are not answered even though answers frequently exist (~55-70%) in published resourcesi ii iii iv.
A large number of practice guidelines have been published, but studies have shown that many of them are not followedv vi vii viii ix.
On the other hand, the American Board of Medical Specialties (ABMS) and the American Colleges of Graduate Medical Education (ACGME) have designated Medical Knowledge as a physician core competency for quality patient care. The ABMS and ACGME define competency in this area as the ability to demonstrate knowledge about established and evolving biomedical, clinical and cognate sciences and their application in patient care. The Institute of Medicine (IOM) has identified five competencies for healthcare professionals, one of which includes employing evidence-based practice (integrating best research with clinical expertise and patient values for optimum care, and participate in learning and research activities to the extent feasible).
This CME program provides physicians with an online information resource (Clin-eguide) where they can find immediate answers to their pressing clinical questions through specific searches of an online information database. The database content is drawn from a variety of sources including the Clin-eguide evidence-based guidelines, the Facts & Comparisons drug information, the 5-Minute Clinical Suite, MEDLINE and the National Guideline Clearinghouse. This resource is delivered over the Internet, which is a convenient medium that allows physicians to obtain real time answers to clinical questions from wherever they are. Physicians are thus able to gain added clinical knowledge—improving their competency and, hopefully, providing better health care to their patients.
A previous incarnation of this CME program as an American Medical Association pilot program (SKOLAR MD) demonstrated that in the context of an online information resource, physicians frequently found the answer they needed and would apply it clinicallyx.
Participation in this program involves identifying a learning objective, researching the topic in the Clin-eguide product and completing an evaluation assessment questionnaire related to what was learned, how it will be applied to practice and the quality of the activity. Answers to those questions will be reviewed, assessed and utilized to improve the overall program and, specifically, the database content.
By participating in this activity, physicians should improve their ability to diagnose, manage or treat patients. Specific learning objectives are determined by participants.
